Congratulations on your pregnancy! In order to match you with a suitable team of midwives, there is some information we request from you. You may choose to fill out this online form, or to call our Clinic Coordinator Penny at 604-874-7999. Whether online or by phone, our midwives will review your information and our office will advise you on whether we have space available within a week. Your information will be read only by our clinic coordinator or the midwives in the practice and will not be disclosed as per our confidentiality agreement.
